Application for Admission

To request an application by mail, please email us at udtsadms@dbq.edu.

  • provide a completed application form (For electronic application CLICK HERE)
  • provide a five- to seven-page narrative statement, reflecting your faith development and views of the ministry. You may email that document to: UDTSAdms@dbq.edu It should be in one of the following 2 formats: .doc or .rtf.
    (If you have questions about these file types, please contact SemTech1@dbq.edu)
  • provide three letters of recommendation (Letters of Recommendation Forms) from the following persons:
    • applicant's pastor
    • a professor (or, if out of school for over 5 years, a person who can comment on applicant's ability to complete master's level work)
    • a friend or colleague
  • be interviewed by a member of the admissions committee either by phone or in person
  • have an official transcript sent from each college, university or seminary attended; and letter of academic standing (Academic Standing Form)
  • submit a $30 non refundable application fee (Fee Payment)
  • upon acceptance submit a non­refundable enrollment fee of $100 (Fee Payment)
